Your First Visit

Your doctor has prescribed physical therapy—what now?

You have the right to choose your physical therapy center. Comprehensive Physical Therapy Center is the right choice. Our staff is dedicated to making your rehabilitation experience as pleasant and productive as possible.

Call for an appointment—

Decide which office is most convenient for you. On our Locations page you will find maps and driving directions to each of our offices as well as hours and phone numbers. When you call, our trained staff will gather all pertinent information and schedule your evaluation as quickly as possible.

Before your visit—
 
What to wear—
  • Please wear or bring clothes appropriate for your condition. If you have a leg problem, shorts are recommended. Comfortable clothing is recommended for the back, neck and shoulder areas.
  • CPTC does have gowns available for these body areas. *Please note that the therapist must be able to see and touch the involved area to give proper treatment.
 
What to bring—
  • Doctor referral/prescription for physical therapy.
  • Insurance Card – CPTC provides billing to your insurance company, but you will be responsible for any co-pays at the time of service. See FAQs for more information.
  • Completed Patient Information Form (PDF) if you have it.
 
Minors and Special Needs Patients—
  • A parent or guardian must accompany minors (under 18) on their first visit to insure that all patient and insurance information is correct and to provide the necessary consent for treatment.
  • It is helpful to both patient and therapist if the parent or guardian understands the treatment plan and what is expected.
  • Parents of younger children are encouraged to remain during treatment.
 
What will happen—
  • On your first visit, one of our licensed Physical Therapists will interview you and gather information on the history of your condition.
  • The therapist will perform an evaluation specific to your condition to determine the most appropriate course of treatment.
  • Prior to initiating treatment, the therapist will discuss any treatment options with you, as well as your goals for therapy.
  • Your first treatment may consist of home instructions, therapeutic exercises, education about your condition and hands on therapy. It may also include the use of modalities such as electrical stimulation, ultrasound, heat or ice in addition to other approaches appropriate for your condition.
